In planning for our patient’s health care needs, we, at Pride Medical,
Inc., have incorporated pharmacy services in our practice.
The Pride Medical Pharmacy is located in our office for your convenience.
Most prescriptions can be filled by in our pharmacy. Our Pharmacy Staff is available to answer questions and provide information about your
prescriptions. Medications filled in our pharmacy may be billed to your
insurance company directly. You will be responsible for any co-payment or
deductible amounts not paid.
We are pleased to announce the expansion of our Pharmacy Network. We

Medical Pharmacy is now a participating provider with Blue
Cross/Blue Shield in the WELLPOINT Pharmacy Drug Program, Express
Scripts, AEtna Pharmacy Management, MEDCO Health Solutions (most
plans), Community Health Plan of Georgia (for State Employees and
Medicaid), and Caremark . We will be pleased to fill your
prescriptions upon presentation of your Pharmacy Drug Card and
payment of your co-pay. Your co-pay must be paid at the time you
pick up your medications.
We also offer convenient free overnight
delivery for most drug plans.

The pharmacy is presently open from 11:00 a.m. to 5:30 p.m. Monday through
Thursday, and from 11:00 a.m. to 4:30 p.m. on Friday. Prescriptions may
be called in by dialing 404.355.3788, x311.
*The pharmacy at Pride Medical is owned by its corporate shareholders (Lee
Anisman, MD; David Morris, MD; and Jean Robinson, PharmD). There is no obligation for any patient to use the Pride
Medical pharmacy. We will happily call or fax your prescription to the
pharmacy of your choice.

As passed by the Georgia General Assembly, the "Patient Self-Referral Act
of 1993" (O.C.G.A. 43-1B-1, et seq.), as amended, requires that health
care providers must notify patients in writing of any investment interest
that the provider has in certain health care facilities or services to
which he or she may refer patients. The Georgia General Assembly
recognizes that such referrals may represent a potential conflict of
interest.
You the patient, have the right to obtain healthcare services or items
from any facility of your choice, unless forbidden by law, including but
not limited to those, listed in this letter in which your provider has an
interest.
